Obesity Medicine 2024 - Strengthening Tertiary-Primary Care Partnerships (Recorded)
In this session, co-presented by a primary care and an obesity-focused tertiary care provider, we will review barriers and facilitators to pediatric obesity treatment in primary and tertiary care settings in the context of the 2023 AAP Obesity Guidelines and the OMA Pediatric Clinical Practice Statements. We will discuss existing models of collaborative chronic disease management between primary and specialty teams (e.g. pediatric asthma) that can serve as prototypes for obesity. We will also provide practical resources/strategies to facilitate strong partnerships between primary and tertiary care settings.
